A Clinical Observation of the Hepatic Abscess

Abstract
A clinical observation was done on 115 cases with liver abscess who were admitted to National Medical Center, Seoul, Korea from January, 1967 to August 1976.
The results were summarized as follows;
1. Male to female ratio was about 2¢¥: 1 and the peak of age incidence came in 3rd decade, followed by in order of 4th decade and 5th decade. "" %
2. Among 115 cases with the hepatic abscess, 99 cases (86.1%), were located in the right lobe and 10 cases (8.7%) were in left lobe and 6 cases (5.2%) were multiple abscess. Pyogenic abscess consisted of 49.0% off all cases, amebic abscess 31.3% and mixed type 19.1%.
3.. Theme most common symptoms of abscess in right lobe were fever, chill and pain on RUQ. But in left lobe abscess, the epigastric pain was the cemmonest symptom. Tenderness on RUQ, jaundice and abdominal rigidity were predominant in the abscess of right lobe but those signs were rarely seen in the abscess of left lobe.
4. Anemia (below lOgm% of Hb) was present in 40% and leukocytosis was more frequently found in pyogenic liver abscess. In liver function test, elevated alkaline phosphatase was present in 93.4% but there was. no difference between location and type of liver abscess.
5. In chest roentgenogram, elevated right diaphragm in the abscess of right lobe was found r in 50%, but elevation of left diaphragm was noted in 40% of abscess in left lobe.
6. In 96% of cases (37), suggestive cold areas were noted by liver scan.
7. The subphrenic abscess was the commonest complication and pyothorax, atelectasis followed in order of frequency. Those complications occured more frequently in abscess of left lobe (7 complication among 10 cases) and less frequently in that of right lobe. Incidence of all complication was 34.8%.
8. The total motality rate was 13%. Prognosis was worst in multiple abscess and pyogenic abscess was worse than amebic abscess.
